• Kind of insurance (Medicare, Private Company) if applicable.
• Insurance plan / Group / Policy number
• Subscriber identification number or Medicare / Medicaid number.
• Patient’s full name as it appears on the Medicare / Insurance card.

We are able to electronically bill most insurance carriers including: Medicare, Medicaid, Caresource, Molina, Anthem, United Health Care, Aetna, Health Reach, Cigna, Medical Mutual, Bureau of Workers' Compensation and others.
